  Thank you for sharing your goals and preferences with us in preparation for your complimentary tennis consultation and level assessment. 

  Please complete the questions below that will help us prepare for your personal consultation. We look forward to getting you started in tennis at ORC!

  Upon submission of this form you will receive a appointment response within 24 to 48 hours.

  For your consultation we will be doing an on court and off court session that will be approximately one hour.  Please come dressed in your tennis attire and with your racquet (we will supply you one if needed). Check in at the front desk on your arrival and we will meet you there at the scheduled time.
